The Care Coordinator is an integral member of the interdisciplinary team, with particular expertise in the area of discharge planning and the other psychosocial environmental aspects of patient and family dysfunction. Demonstrates competency with interventions and clinical judgment based on the population and age specific needs.
Master’s degree in a psychosocial science is required. MSW and licensure preferred. Knowledge of group process, family systems, development stages, mental health systems and mental health regulations required. Analytical, planning and decision-making skills are necessary to effectively make accurate and rapid judgments in critical situations. One – two years of related experience required. Basic computer skills required. PRN (Monday-Friday day shift) positions available.
