Community Healthcare Worker

Overview: Help people live happy, healthy lives through work as a Community Healthcare Worker (CHW). Responsibilities

Key Functions/Responsibilities

Builds relationships with assigned members through in person, phone, and written communication to understand their concerns, needs, and goals, and works toward collaborative goal setting.
Works with disparate populations to increase involvement in personal healthcare decisions, guide members to advocate for more care, and educates as to the availability and importance of preventive care.
Conducts health risk assessments to identify member needs and provides education and support.
May attend community meetings to understand community issues or to build relationships with community members.
Meet members at physician appointments, in the community, and in the members’ homes to support involvement in personal healthcare decisions. Assists members with culturally appropriate Social Determinants of Health (SDOH) needs.
Remain up to date on knowledge of community resources relating to various areas of specialty appropriate to the needs of members/families through participation in ongoing trainings, informal learning opportunities, supervisor mentoring and general professional development opportunities.
Data entry and documentation of interactions with members.

Qualifications

Skills/Abilities:

Knowledge of community health resources and social determinants of health
Ability to effectively communicate with a diverse population and establish trust
Effective active listening skills
Bilingual in Spanish (speaking, reading, and writing) a plus, but not required
Experience using Microsoft Office and data entry

Qualifications and Education Requirements

High school diploma or GED required. Associates Degree in Human Services or other correlated field preferred
Two (2) years of work experience directly related to the duties and responsibilities or equivalent combination of education, training, and experience preferred
Certified Community Health Worker; Or ability to pass CHW Certification process with enrollment in required trainings scheduled within nine (9) months of hire.

Physical Requirements & Conditions of Employment:

Valid Driver’s License and ability to drive safely to client and community locations throughout the day
Ability to walk limited distances outdoors and climb stairs to meet with members in their home or community environment
Ability to communicate verbally in person, over the telephone and in writing

Travel

Significant amount of travel with 80% of the time on the road utilizing your own reliable transportation to travel throughout day to member meetings.
