The Journeys Program is looking to hire a new LCPS!

The Journeys Program is a new and unique program that offers support to individuals with mental health diagnoses and physical disabilities. The LTSS – Certified Peer Specialist (LTSS-CPS, or LCPS for short) provides flexible, community based services that are designed to promote the empowerment, recovery, and community integration of individuals who have mental health challenges, as well as physical disabilities.

-Works with individuals to identify, develop, and access supports to increase his/her success in community integration

-Supports and teaches recovery and recovery tools and models personal responsibility, self-advocacy, and hopefulness

-Partners with each individual to assess their hopes, strengths, accomplishments and challenges in order to achieve his/her stated goals

-In partnership with program participant, the LCPS develops the recovery plan and their support system in order to support them in becoming self-sufficient

-Supports individuals in the self-management of critical or crisis situations.

-Supports individuals with transitions to independent living


-Minimum of a High School or GED Diploma

-Associates Degree in a Human Services related field is preferred

-Credentialed as a Certified Peer Specialist

-Experience in a behavioral health/LTSS work environment

-Knowledge of behavioral health and long-term support and services systems and resources

-Excellent communication skills (written and oral)

-Strong time management and organizational skills
