PHMC’s Mental Health Outpatient Program is looking for a full time substance abuse and
mental health therapist to provide clinical treatment in our mental health and substance abuse outpatient program

Responsibilities

• Attend daily staff meetings and inform staff of any important resident information.
• Implement recovery process for residents as defined PHMC’s Mental Health Outpatient program goals and philosophy.
• Provide continuity of care as defined by treatment plan for each consumer.
• Lead group therapy sessions.
• Conduct individual therapy sessions with each consumer on your caseload.
• Conduct individual therapy sessions as needed by consumers or as indicated by observations.
• Work collaboratively with the clinical team
• Coordinate and collaborate with off-site, community-based providers.
• Alert Clinical Director to any current or potential behavioral problems of residents.
• Meet or exceed program productivity requirements.
TREATMENT PLAN:
• Devise, with consumer and treatment team, within 15 days, a comprehensive treatment plan based on Psychological Evaluation and other psychological assessments.
• Update treatment plans.

PROGRESS REPORT:
• Maintain continuous record of all contacts, events, and progress toward accomplishment of goals as stated in Treatment Plan.
• Record individual and group progress notes in the DAP format after each therapeutic activity.
• Complete reviews in compliance with state licensing and other program guidelines.
• Maintain charts that meet all standards.

CONTINUING CARE PLAN:
• Assist treatment team and consumers with the information of continuing care plans
• Other duties as assigned.

Qualifications

• Master’s degree and two years related experience with one of the following credentials: licensed in PA as a Professional Counselor, Clinical Social Worker, Marriage & Family Therapist or Psychologist; Certified Alcohol and Drug Counselor (CADC) or Certified Advanced Alcohol and Drug Counselor (CAADC) .
• Competence in individual, group therapy, and family intervention.
• Experience and training in evidence-based approaches.
• Thorough knowledge of alcoholism and other addictions with the particular attention of the needs of dually diagnosed consumers.
• Thorough knowledge of mental health disorders and treatment.
• Ability to use therapeutic skills, which will assist consumers in process of recovery.
