The Certified Recovery Specialist (CRS) has experience with behavioral health services and is willing to use and share his or her lived experience and first-hand knowledge to hospital patients and their family members. The CRS engages with persons served to create warm hand offs between acute inpatient treatment and other levels of care classified as complex chronic care management, and creates rapid access to drug and alcohol or mental health services to improve adherence to treatment recommendations.

Responsibilities

1. Engage in brief relationship building and active listening with clients receiving treatment in medical inpatient setting.
2. Provide substance use screening, brief intervention, and referral to treatment and/or other applicable community resources and services.
3. Provide supportive (peer-based) coaching and encouragement to clients contemplating or actively seeking help with substance use treatment and recovery.
4. Provide education to enable clients to identify their relapse symptoms or relapse triggers as well as their protective factors (to avoid relapse).
5. Outreach, engage, and provide education to family members, other natural supports, and other professionals regarding substance use disorders and available programs/resources.
6. Assist in after-care planning for individuals discharging from medical setting and help ensure that behavioral health needs are included in care plan.
7. Engage in team-based care and work cooperatively with other medical professionals and county-wide personnel.
8. Serve as a direct care navigator and liaison to Penn Foundation’s community-based and site-based programs (in addition to general community programs and resources).
9. Document each service-related contact in case note according to program standards.

Qualifications

• Education/Training: Minimum high school diploma or equivalent. Certification as a Recovery Specialist required upon hire (or completion and passing of CRS training/exam within six months of hire).
• Job Knowledge: Understanding of principles of mental illness and recovery.
• Certification: Maintains Certification as a Recovery Specialist and adheres to the
profession’s and Penn Foundation’s Code of Ethics.
• A Pennsylvania Criminal Record Check, a Pennsylvania Child Abuse History Clearance and a FBI Criminal History Background Check.
