The Registered Nurse cares for the physical and behavioral health needs of population with serious mental illness (SMI). This position is an active member of the multi-disciplinary team that provides therapy, case management, and support to our consumers.

Essential Job Functions
1. Completes both psychiatric and physical health assessments for ACT Consumers.
2. Works collaboratively with the psychiatrist and community physicians in meeting the health and behavioral health needs consumers.
3. Administers medication including injectable medication.
4. Provides consultation on medication health issues and collaborate with the psychiatrist and other RN’s in the management of medications.
5. Provides crisis intervention services in office and in the community. Provide emergency on call phone support to ACT team members as needed.
6. Provides timely, accurate documentation.
7. Participates in daily organizational meetings.
8. Participates in the development of policies and procedures related to health, medication, and psychiatric care.
9. Manages pharmaceuticals and medical supplies.
10. Educates Consumer and team members regarding substance abuse and psychiatric medication interactions.
11. Assists in the overall team case management, counseling, and support functions.
12. Provides assistance and support to the psychiatrist/physicians in meeting the overall health and behavioral needs of clients receiving psychiatric evaluation and medication management including prior authorizations for medications as needed.
13. Communicates regularly with other medical staff and clinical staff regarding patient health care and medication needs.
14. Works closely with insurance companies, funding sources, pharmacies, laboratories, and hospitals related to patient medication concerns or discharge planning.
15. Educates, counsels and supports patients in relation to their behavioral health and physical health and wellness care. Reinforce policies and procedures that promotes the behavioral and physical health and wellness of patients.
16. Provides in-home, community, hospital, and transitional care supports/interventions to patients involved in project as needed.
17. Maintains accurate and timely information in the electronic health record (EHR) and support the mission of meaningful use measures.
18. Serves as a consultant to clinical staff on medication, health, and safety concerns.
19. Provides staff training on health issues, including HIV/AIDS/TB and other infectious diseases to the organization’s established group programs.
20. Serves on BCC’s committees in regard to health and safety needs.
21. Provides timely response to patient’s calls regarding medication issues, problems, concerns, or questions.
22. Assists clinic operation with attaining patient’s Temperature, Pulse, Respiration, Weight, Body Mass Index (BMI) and injections.
23. Flexible hours – morning/afternoon (early evening) shifts. There is an expectation of flexible work hours based on client needs. Includes some weekend shifts as needed. Includes on call duties as needed.
24. Demonstrates good working knowledge and use of technology.
25. Performs all other related duties and responsibilities as assigned.

Physical Requirements
SEEING: Must be able to see well enough to read reports and use computer.
COMMUNICATING: Must be able to communicate well enough to clients, vendors and coworkers.
MOBILITY: Must have mobility to move about the agency. Must have mobility to move about the community.
LIFTING/PULLING/PUSHING: Must be able to lift up to 25 pounds.
OTHER: Must be able to write, type and use technology.

Qualifications

Licensed Register Nurse, active in the state of Pennsylvania required. At least one (1) year experience in a behavioral health setting required. Valid PA Driver’s License required. Bilingual: English/Spanish a plus.
