Under the general supervision of the Program Director, contributes in a peer support capacity to facilitate recovery and enhance wellness by sharing his/her personal/practical experience and first-hand knowledge to the team and clients. Serves as a role model for recovery for both staff and peers and partners with peers in developing recovery tools and building support networks.

Responsibilities

1 Assists designated peers with rehabilitative social support, advocacy, educational and vocational mentoring and self-maintenance
2 Provides information on accessing community resources.
3 Assists in transportation for peers to necessary appointments, support and outreach.
4 Provides reminder calls to designated peers to offer reminders and verbal supports in relation to their recovery.
5 At the request of the peer, assists family members in obtaining support resources, as well as educational information regarding recovery tools.
6 Provides supports to individuals in recovery in the community when needed.
7 Provides marketing and trainings within the community as needed.
8 Partners with peers to create and attain goals on the Individual Recovery Plan.
9 Completes necessary documentation for each peer encounter (i.e. service activity, contact sheets, etc) within 48 hours of encounter.
10 Completes all necessary paperwork during the intake, and updates paperwork as needed.
11 Completes monthly log of services provided.
12 Meets with team on a bi-weekly basis for staff meeting and peer reviews.
13 Assists Peers with creating an Individual Recovery Plan. Initial IRP must be completed within 30 days of first face to face visit. A six month review from the date of the most current IRP will be required for continuation of services.
14 Reviews Individual Recovery Plan with peers every 6 month or earlier as needed.
15 Acts as a liaison with other community agencies/groups, funding sources, and businesses that relate to the services provided to peers.
16 Acquires knowledge or resources in assigned county/counites while maintaining an excellent working relationship with all providers.
17 Provides onsite visits to other community agencies in order to educate them about the peer services available.
18 Communicates on an as needed basis with providers regarding issues relevant to continuity of care.
19 Provides after hours telephone support to enrolled peers as assigned.

*Position is Grant-funded and therefore time-limited

Qualifications

Required Skills
Excellent human relations and oral/written communication skills
Basic computer skills including email, Word, and internet
Knowledge of and ability in the care and support of individuals in a specific age group
Requires a valid driver’s license in the current state of residence
Required Experience
Must have High School diploma; Associate's Degree preferred

Must have Recovery Support Specialist Certification

Must be a self -identified recovering consumer; experience in navigating the recovery system (i.e. crisis services, vocational rehabilitation, recreational activities, and human services, etc.); A history of coping with own substance use disorder issues and maintenance of own recovery; Medical Assistance or related health field

Must have 12 months of paid or volunteer experience within a three year period
