- Download and complete the Recertification Application.

If you would prefer to use a print version of the application to recertify you can download the Recertification Application as a PDF. This PDF can be typed in, saved, then emailed, mailed or faxed to PCB. Please choose only one way to submit the application. Contact information for PCB is located in the application.
