Complete an intake packet on new referrals (peers). Complete a Strengths Based Assessment, Individual Service Plan and Continued stay on peers actively enrolled in the Peer Support Program. Work on established goals with peers. Assist peers with accessing community based resources. Document services provided using recovery language in an electronic record. Meet weekly with a supervisor and twice a month in group meetings.
