Serving Transition Age Youth aged 14-26 in Bucks, Montgomery, and Delaware Counties , PA. Interested Individuals must have or be willing to obtain PCB approved CPS Certification. CFF will assist in obtaining certification training, if needed.

Job Overview:

A CPS will serve as an advocate and mentor to Transition Age Youth (14-26) with serious mental illness and/or emotional disturbance to develop community supports, navigate MH systems and access resources, support maintenance of wellness and recovery based on personal lived experience, and INSPIRE HOPE to help youth reach their full potential and overcome obstacles in the face of life’s daily challenges. A CPS will not only assist in the identification of individualized goals, but also walk the young person through the steps to achieve success with their goals.

A CPS will meet weekly in the home and community settings with the young person offering direct support with a focus on:

Personal wellness and the use of coping skills.
Building social skills and a social community.
Supporting educational and/or employment opportunities.
Improving functioning in their environment.
Developing daily living skills.

Job Responsibilities:

Provide peer counseling and support to youth seeking to enhance their quality of life.
Encourage and support the maintenance of wellness and recovery through mentorship and the installment of hope, empowerment, and self-advocacy.
Identify community and natural supports and assist youth with accessing and participating in such activities or events.
Use one’s own recovery experience to facilitate the achievement of a youth’s individualized life goals.
Assess and support ongoing progress with service plans.
Guide youth in making necessary changes and celebrate successes.
Model and promote skills required for employment, community integration, and independent living.

Job Functions:

Engage in weekly face-to-face sessions with each youth on caseload.
Meet youth where they reside, in the community, and/or wherever services are most appropriately delivered.
Participate in weekly individual Supervision.
Attend required trainings to maintain CPS Certification
Use office equipment (phone, fax, copier, and scanner).
Document session activity through electronic health record via progress notes and program specific paperwork by typing.
Ensure caseload files are organized, and paperwork is completed within required deadlines.
Maintain a varied work schedule, including evenings.
Work both individually and in teams with colleagues.
Effectively communicate and interact with youth, peers, and community representatives.

Qualifications

Qualifications:

Have High School Diploma or GED.
Be certified as a peer specialist (CPS) through the PA Certification Board.
Maintain, within the last 3 years, at least 12 months of successful full or part time paid or voluntary work experience or 1 year of post-secondary education experience totaling 24 credit hours.
Maintain valid driver’s license, insurance, and vehicle.
Engage in local travel in your own vehicle to offsite locations as required by caseload, supervision, and/or trainings.
Transport youth to various community locations, as needed.
Exhibit stability and competency in personal wellness and recovery and use of coping skills.
Be able to reflect the culture and understand the values of the Transition Age Youth population.
Demonstrate proficiency in reading, writing, and computer literacy.
Be able to work in a drug free workplace.

Preferred Qualifications:

Demonstrate excellence in communication.
Possess knowledge of community resources in the county in which you work and surrounding area.
Demonstrate proficiency in organization and time management.
Demonstrate the ability to assist youth in developing empowerment skills and combating stigma through self-advocacy.
Possess knowledge and skill to teach basic problem-solving strategies and engage youth in self-directed recovery.
Possess knowledge and skill to help youth identify community resources and develop life skills necessary for independent living.
Possess knowledge and skill to help youth establish and participate in self-help and educational groups that support their wellness and recovery based on their strengths and interests.
