Job responsibilities include mentoring, advocating and providing peer support to individuals in their journey of recovery. The individual is responsible for maintaining client charts.
Essential Functions/Accountabilities.
1. Strives to meet the mission and goals of the agency, which are mental health recovery.
2. Works with clients to assess, develop, implement and recommend modifications to a service plan.
3. Raise awareness about recovery-oriented services.
4. Reenforce knowledge, attitudes and behaviors.
5. Provide assistance and encouragement in accessing appropriate services to maintain living, learning and working with the community.
6. Educate community providers, family members, and consumers about recovery-oriented services.
7. Teach skills needed to make informed life decisions, increase life management skills including decision making and problem solving.
8. Performs associated administrative functions, as appropriate, such as submitting billing documentation as required.
9. Documents all client encounters and contracts made on behalf of the clients; write progress notes.
Management of work processes including planning, time management and reporting.
Use of financial and other resources efficiently and effectively.
Communication (includes active listening skills and responsiveness.
Professionalism includes practice of confidentiality and privacy standards as well as use of agency's information technology.
Promote and demonstrate the Mission, Vision and Values of Grapevine Center, Inc.
High school diploma or GED, strong interpersonal and communication skills and the ability to work effectively with a wide range of constituencies in a diverse community. Ability to work independently and as a team. Current PA Driver's license, Act 33/34 clearances, Background check. Self-identified as a current or former mental health consumer or co-occurring consumer who can relate to others with similar experiences. Completed the Certified Peer Training approved by OMHSAS
