This position provides advanced level administrative support across various programs. Activities include data collection and reporting, surveying, overseeing the coordination of care across programs, CQI activities, participating in huddles, and scheduling and assuring patient access to care. The Coordinator will work closely with his/her Direct Supervisor and collaboratively with other Department Supervisors within the agency.

Clinical Care Coordination is provided to the following departments: Assertive Community Treatment (ACT), Case Management, Clinical, Integrated Medical, and Wellness..

Essential Job Functions
1. Provides outreach and a high level of customer service to referrals sources and collaborative partners.
2. Provides direct administrative assistance to departments noted above.
3. Participate in overall compliance with all CCBCH, insurance, licensing and other standards and relevant laws/regulations that govern clinical care.
4. Supports internal operations within and across programs.
5. Assists with the accuracy, timeliness, and compliance of information contained in the electronic health record (EHR).
6. Keeps up to date on standards and regulations attending trainings, webinars, and community events on same.
7. Assists with clinical screenings and patient surveys.
8. Provides active outreach to clients for no shows, routine follow up, re-engagement, and identifying and resolving barriers to maintaining care.
9. Participates in daily huddles sharing relevant data and facilitating patient access to care.
10. Develop an understanding of trauma and how it impacts others.
11. Incorporate trauma informed practices into job duties and responsibilities, including interactions and communications with others.
12. Be an active member of BCC’s culture of caring through positive and engaging social interactions.
13. Be proficient in the use of Microsoft Office, EHR, and general office technology.
14. Conduct standardized pre-screenings.
15. Schedule and present at treatment plan meetings.
16. Performs all other related duties and responsibilities as assigned.

Qualifications

As of 11/08/2021, employment consideration will be given only to vaccinated candidates.

Minimum Qualifications
Associates Degree plus at least one (1) year of experience working in a health or behavioral healthcare setting or in a similar capacity. Bilingual (English/Spanish) preferred. Knowledge of or experience with trauma-informed approaches preferred.
