The Recovery Care Coordinator (RCC) will work in partnership with individuals with substance use disorder (SUD) who may benefit from wrap around care coordination (including but not limited to substance use treatment, physical and mental health services, and other social services) and recovery support services, whether at the drop-in recovery center, through technological means, or in the community. The RCC will serve as a role model, mentor, advocate, navigator, and motivator to individuals in or seeking recovery in an effort to promote recovery planning and goal realizations, which may include reductions in harm from substance use and sustaining long-term recovery. The RCC must demonstrate an ability to facilitate care coordination and recovery planning that follows diverse recovery pathways (including medication and moderation), consciously share personal recovery experiences as appropriate, develop authentic peer-to-peer relationships, and have an understanding of and respect for each individual’s unique path to recovery. In addition to these responsibilities, the PRCC will also serve as a community liaison to referral partners in the community that provide wrap around services to individuals engaged in services.
Provide peer-based recovery support services and care coordination to individuals contemplating or actively seeking help with behavioral health recovery.
Engage in relationship building with individuals to establish rapport, as evidenced by empathetic and active listening and communicating in a non-judgmental way.
Participate in daily shift debriefs, as well as weekly individual supervision, and monthly group supervision.
Facilitate consistent engagement and connection with all program community and service referral partners.
Administrative duties to include care coordination plans, recovery plan creation, data tracking and entry, and regular digital communication via e-mail, group messaging, etc.
Conduct self in an ethical manner by adhering to the PCB codes of ethics.
Follow and adhere to the recovery bill of rights.
At least 1-year experience facilitating wrap around care coordination / recovery case management for individuals with a history of substance use.
Background check may be required for staff that will work with youth, inside criminal justice settings, or other settings as determined by the Executive Director.
