Provide individual, family or group counseling to persons with co-occurring substance abuse and mental health problems.
Directly assist patients in their daily efforts to build personal recovery skills and habits for self-determination and self-reliance, including use of harm reduction model interventions.
Organize and facilitate substance abuse groups offering patients new, healthier ways to overcome recovery roadblock and develop self-esteem.
Facilitated patient referrals to self-help groups (NA, AA, etc.) and sponsors who are accepting of individuals with co-occurring mental health diagnosis.
Participate in the completion of a comprehensive assessment of drug and alcohol use and the effects of the addition on the individual’s physical, social, and emotional health.
Participates in and conducts the patient-centered comprehensive assessment of psychiatric history, mental status, diagnosis; physical and dental health; drug and alcohol use; education and employment; social development and functioning; ADL’s; and family system and relationships.
Provides direct clinical services to patients on an individual or group basis in the office or community settings to teach behavioral symptom-management techniques to assist patients to adapt to and cope with internal and external stresses.
Provides care coordination, including developing, writing, implementing, evaluating, and revising treatment goals, providing individual supportive therapy and symptom management, and supporting patients’ families.
Provides practical help and side-by-side coaching, problem-solving, training, and supervision to help patients obtain the necessities of daily living which may involve the use of staffs’ personal vehicles.
Provides ongoing assessment of symptoms and response to treatment and make appropriate changes to treatment plans to ensure that appropriate interventions are provided in response to the patient’s needs.
Plans and facilitates social/leisure activities to improve the social skills of patients, as directed by the Team Leader.
Provide symptom education to assist patients in understanding their mental illness and to identify relapse symptoms.
Maintains medical record documentation that is accurate, complete, comprehensive, legible, and timely.
Maintains medical record documentation that meets the legal, professional, and ethical standards of practice for clinicians, as well as standards outlined by Joint Commission and MCO’s.
Participates in the on-call system according to WellSpan Philhaven CTT policy.
