The Community Health Worker (CHW) serves as a bridge between ChesPenn and our patients and their families to help improve their health. The CHW uses his/her knowledge of the local community and the health and human services resources in the area to empower patients and their families to understand and mange their health needs.

Responsibilities

Help patients develop health management plans and goals
Coach and coordinate patients in the management of their chronic health conditions and self-care
Establish positive, supportive relationships with participants and provide feedback
Facilitate communication and client empowerment in interactions with health care/social service systems
Facilitate communication and coordinate services between providers
Work effectively with stakeholders (patients, staff, partner agency representatives, etc.) from diverse backgrounds in reducing cultural and socio-economic barriers between clients and institutions
Continuously expand knowledge and understanding of community resources, services and programs provided
Identify and apply appropriate role definition and boundaries

Qualifications

High school diploma or equivalent
Current COVID-19 and flu vaccines required
A valid driver's license

Some experience in health care and/or community engagement or previous training in a CHW course a plus
CHESPENN WILL PROVIDE TRAINING - NO EXPERIENCE NECESSARY if requirements are met and interested in learning

Current or former resident of Chester and vicinity, strongly preferred; experience in community/outpatient settings a plus
Genuine passion and desire to enter a helping profession in health care or social services
Ability to successfully complete a Community Health Worker curriculum
Bilingual/bicultural fluency a plus
