We are seeking a Certified Recovery Specialist/Certified Family Recovery Specialist to join the Family First Health team.  Are you looking for a way to use your life experience to help others? We are seeking a Drug and Alcohol Care Coordinator to support patients receiving MAT with Family First Health. This a great opportunity to use your life experience and personal growth as an integral tool in patient support. Must work well as a team member collaborating with other professionals for patient care, as well as independently.  If you are a Certified Recovery Specialist/Certified Family Recovery Specialist, or meet the requirements to pursue such with the Pennsylvania Certification Board, this could be a great opportunity for you to use your experience to help others in a unique professional role. Pennsylvania Certified Recovery Specialist/Certified Family Recovery Specialist certification is a requirement for this position.

ESSENTIAL DUTIES AND RESPONSIBILITIES   

Perform work associated with therapeutic care, case management, recovery support services, treatment and rehabilitation of persons with drug and/or alcohol use through care coordination.  Services and support will be delivered to substance use patients in the context of Medication Assisted Treatment (MAT).    

In accordance with Patient Centered Medical Home Philosophy, collaborates with team members including:  patient; primary care provider; patient care coordinator; consulting psychiatrist; and others to deliver comprehensive care to patients of Family First Health through:

Patient Engagement and Empowerment

Participates in case coordination by utilizing case management tools, such as strength-based case management, motivational interviewing and home visits to facilitate patient retention in care and move patients toward independence and increased autonomy.

Assists participants in the management of their recovery by ensuring that the resources to address the participant’s needs are in place.

Provides drug and alcohol education, risk reduction counseling, and medication adherence counseling. 

Promotes participant retention, self-sufficiency in treatment and utilization of available support through educating participants about service availability.

Engages in brief relationship building and active listening with patients receiving treatment

Provides supportive (peer-based) coaching and encouragement to patients contemplating or actively seeking help with substance use treatment and recovery.

Assists with participant transition from professionally directed service plan to self-directed Recovery Plan to facilitate long-term recovery maintenance through on-going peer-based coaching.  

Will serve on-call duties based on on-call policy and departmental procedures.

Connection with other providers

 Identifies, develops and maintains positive linkages with agencies and social service organizations/services in the community and within FFH. 

Communicates with FFH interdisciplinary team, peers in other departments, managers, executives, patients and families, and other community agencies to ensure ongoing care for patients, to maintain connectedness, and for positive internal/external customer service. 

Facilitates warm handoffs between all available behavioral health and physical/dental health treatments. 

Creates and manages long-term individualized service plan by assessing patient’s target needs and reassess on timely basis, including making appropriate referrals for treatment and rehabilitation of drug and alcohol dependent patients. 

Documentation 

 Maintains timely and accurate documentation and tracking of all service provision, evaluation, and statistical data for the program; including detailed patient demographics, medical information, risk factors, and other information needed related to grant funding. 

 Understands and adheres to all state and federal drug & alcohol confidentiality laws and related statutes. 

Attends and actively participates in both internal and external meetings as required. 

Attend trainings and conferences as determined by supervisor. 

Performs other duties as assigned and related to the position. 

  

 

KNOWLEDGE/SKILLS/ABILITIES  

Minimum qualifications include a preferred bachelor’s degree in social work, nursing/medical, or related human services field preferred or an equivalent combination of education, training, and experience.  Previous education/experience with drug and alcohol programs preferred.  

Thorough knowledge of the Guiding Principles of Recovery.  

Eligibility to obtain a Certified Recovery Specialist certification (CRS) or Certified Family Recovery Specialist certification (CFRS)

Recovery: minimum of two (2) years uninterrupted participation in program. Recovering individuals must be actively involved with a recovery program.

Knowledge of MAT and ability to perform duties in the context of MAT. 

Knowledge of computer software applications (Microsoft Office Applications, others) 

Knowledge of grammar, spelling, and punctuation as it relates to correspondence necessary to perform essential job functions  

Knowledge of basic arithmetic to make calculations, balance and reconcile figures, and make changes accurately

Excellent customer service skills  

Ability to establish rapport with multi-cultural participants 

Ability to effectively establish appropriate boundaries 

Strong written and verbal communication skills 

Skill in operating office equipment  

Ability to critically think which means the ability to think clearly and rationally about what to do or what to believe; understand logical connections between ideas; and identify, construct, and evaluate arguments

Ability to exercise individual judgement and discretion in matters of significance 

Ability to work a varied schedule including evenings, weekends, additional hours, and on-call hours as required by the department 

Ability to work in, and respond to demanding environment 

Ability to communicate well and in a caring and supportive manner with individuals of all backgrounds. 

Ability to work efficiently and accurately with strong attention to detail 

Ability to sort and file materials correctly by alphabetic or numeric systems  

Ability to establish and maintain effective working relationships with patients, employees, and the public  

Ability to be organized, proactive in follow-up and follow-through, and to juggle multiple competing tasks
