We are in collaboration with a Philadelphia heath plan to improve patient medication adherence to Star Rating medications; high blood pressure, diabetes and high cholesterol medications for their members. This is being accomplished through utilization of a medication managemnt solution called the MedaCube. Specifically improving CMS PDC adherence threshold to these medications to be over 80%. The In-home Community Support Specialist will be responsible for managing all aspects of the MedaCube solution: provide outreach, fill, manage, monitor patient medication adherence and inventory supply in order to improve patient health outcomes, reduce cost and run a successful program. We are looking for a reliable, self-motivated, technology proficient, "Home Tech" to install and support the MedaCube device in patient's homes in Philadelphia, PA.
