We are in collaboration with a Philadelphia heath plan to improve patient medication adherence to Star Rating medications; high blood pressure, diabetes and high cholesterol medications for their members. This is being accomplished through utilization of a medication managemnt solution called the MedaCube. Specifically improving CMS PDC adherence threshold to these medications to be over 80%. The In-home Community Support Specialist will be responsible for managing all aspects of the MedaCube solution: provide outreach, fill, manage, monitor patient medication adherence and inventory supply in order to improve patient health outcomes, reduce cost and run a successful program. We are looking for a reliable, self-motivated, technology proficient, "Home Tech" to install and support the MedaCube device in patient's homes in Philadelphia, PA. Responsibilities

Arrange appointments with Customers to install and then refill their MedaCube.

Determine optimal routing to handle multiple appointments per day.

Drive to customer's home to install and train them on how to use their new medication solution.

Coordinate a schedule with the customer to refill the MedaCube every 30, 60 or 90 days.

Work with the PharmAdva’s Rochester based team to configure, ship and support MedaCubes.

Work with Customer's Pharmacy to ensure timely refills.

Qualifications

Highly proficient and caring, interpersonal communicator.

Driven by community involvement and making a difference.

Highly proficient with learning and using a technology-based product.

Safe driver willing to use own car (reimbursed) for customer appointments.

Flexibility to drive into multiple neighborhoods.

Working knowledge of G-Suite or Microsoft Office (Excel, etc).

Patience and empathy with aging/sick patients.

Good phone communication skills.

Ability to lift and carry the MedaCube (15 lbs).

Bilingual is a plus (Spanish).
