Accomplishes clients' care by using their own unique experiences to teach and support consumers in building skills and resources to facilitate individual recovery, assessing treatment needs; developing, monitoring, and evaluating treatment plans and progress; and facilitating interdisciplinary approaches. This position provides case management to individuals with co-occurring disorders along with recovery support. The IOP Peer Counselor will complete an assessment of the individual’s strengths and needs to work with the individual to complete an individualized strength-based service plan to guide them in overcoming barriers to recovery and towards independence and stability. The IOP Peer Counselor must be able to work with IOP integrated team to collaborate with behavioral health and medical departments to meet the needs of the individual and identify appropriate resources in the community to facilitate the individual’s recovery.

Responsibilities

1. Complete a comprehensive assessment of clients’ skills, strengths, and needs to effectively determine clients who may benefit from case management services.
2. Encourage, support, and assist clients in meeting support plan goals.
3. Assist clients in locating resources, provide referral to those resources, and maintain updated community resources list.
4. Assist clients in overcoming barriers to follow through with services, such as needs for childcare or transportation.
5. Serve as an advocate to help clients overcome barriers and obstacles.
6. Utilize role playing and other skills building techniques as appropriate to maximize client progress.
7. Maintain individual case records on clients receiving services.
8. Complete follow up monitoring of clients after completion of services.
9. Communicate special needs of clients to their primary counselor.
10. Assist with organizing and staffing childcare support.
11. Assist individuals with transition from inpatient facility, including hospital stay and/or rehab facility to coordinate continuance with outpatient services.
12. Develop an understanding of trauma and how it impacts others.
13. Incorporate trauma informed practices into job duties and responsibilities, including interactions and communications with others.
14. Be an active member of BCC’s culture of caring through positive and engaging social interactions.
15. Demonstrates a commitment to diversity, equity, and inclusion.
16. Demonstrates skill in understanding of cultural differences.
17. Effectively assess consumers’ skills, strengths, and needs that will support ongoing substance abuse recovery.
18. Develop and/or have input into the professional development of a treatment plan that will promote recovery.
19. Assist consumers in overcoming barriers to care.
20. Provide in home and community outreach to consumers.
21. Facilitate referral services as necessary.
22. Maintain a philosophy that is supportive of the hope for recovery for all persons and that is inherently focused on consumer strengths and opportunities.
23. Assist substance abuse patients in crisis, needing a higher level of care, or who are in need of further engagement to facilitate follow through with care.
24. Perform all other related duties and responsibilities as assigned.

Qualifications

Associates Degree in the social services or related field required. Lived experience with Certified Recovery Specialist certification preferred or ability to obtain Certified Recovery Specialist certification within 2 years. Experience working with individuals with co-occurring disorders preferred. Bilingual: English/Spanish preferred. Valid PA Driver’s License and the ability to drive, travel and provide community-oriented services required. Knowledge of 12 Step Program, drug and alcohol disorders and treatment, community resources, principles used towards achieving recovery, and Medication Assisted Treatment (MAT) a plus. A willingness to work with integrated team members required. Knowledge of or experience with trauma-informed approaches preferred. Self-identified current or past consumer of substance abuse treatment services with at least two (2) years recovery. May have a dual mental health disorder with at least two (2) years recovery. Flexible schedule that requires some evenings and occasional Saturday hours.
