The Children and Youth Outreach Lead Case Manager provides assessment and referral to the BCCYS Intake Division for individuals who have been identified with high-risk alcohol or other substance use. These clients are referred to BCCYS by medical professionals or other community agencies.

  • Serve as a primary liaison with Bucks County Children and Youth Services (BCCYS) Caseworkers.
  • Ensure consistent communication between the Council and BCCYS Caseworkers and Supervisors.
  • Ensure that services provided are in collaboration with the assigned BCCYS Caseworker.
  • Screen clients for appropriateness for treatment and/or Outreach Services through an initial assessment.
  • Monitor client's participation in recommended interventions and treatment.
  • Provide individual support to clients including engagement in appropriate treatment level of care.
  • Establish and maintain liaison with case management services and Department of Public Welfare to ensure uninterrupted treatment funding where necessary.
  • Meet weekly with the Case Management Services Clinical Supervisor for supervision.
  • Ensure client records are kept in a locked filing cabinet on Council premises.
  • Assure the accurate completion of all records, files, data entry and reports for timely submission.
  • Maintain a caseload of 25 clients for Outreach unless volume of referrals requires a temporary increase with a maximum of 30.
  • Input and upkeep all client records, adhering to all policies and procedures for client contact and documentation.
  • Submit monthly billing and reports in a timely manner.
  • Enter and maintain client status for annual reporting to BCCYS.
  • Uphold policies and procedures for program operations.  Make recommendations for adjustments when deemed appropriate.
  • Other duties as assigned by Executive Director.

 

Qualifications

Qualifications: Master’s degree in Human Services or related field with 2 years of clinical experience in a health or human service agency or a Bachelor’s Degree in a related field with 3 years of experience in substance use disorders, which includes a minimum of 2 years of working directly with the chemically dependent. Must be currently certified by the Pennsylvania Certification Board as a Certified Associate Addiction Counselor (CAAC), Certified Alcohol and Drug Counselor (CADC) or able to obtain certification within 1 year of hire.

Must be able to work well with people, using good communication and program management skills to ensure the smooth operation and success of The Council’s case management programs and maintain professionalism in all internal and external interactions.
