The Certified Peer Specialist is a Full Time role located in Norristown, PA. The pay rate is $16.00 per hour. Full Time staff are eligible for all of our exciting benefits, including health insurance and over five weeks of paid time off!

Trail Guides is a mobile Peer Support program in Montgomery County that specializes in working with young adults ages 18-26 years old that are diagnosed with a mental health challenge. Trail Guides program staff, who are Certified Peer Specialists (CPS), use their shared lived experience with mental health challenges to assist program participants choose their own recovery paths that lead to independent, fulfilling and productive lives in the community while reducing hospitalization, incarceration and engagement in negative and self-destructive behaviors. Some examples of common goals that participants decide to work on include: community integration, employment, housing, obtaining a driver's license, self-care, and improving mental and physical wellness.

Responsibilities

Collaborates with participants with assessing their hopes, strengths, accomplishments and challenges in order to create goals related to self-sufficiency, community integration and community inclusion; assists to identify community and natural supports and resources to achieve goals.
Supports participants in the development of their self-directed goal plans, the self-management of critical or crisis situations, and the coordination of relevant supports; facilitates the participant's self-review of their progress upon each encounter.
Assists participants with educational, employment, and social related goals, benefit acquisition/medical, mental health and substance use services, budgeting skills and exploring housing options in accordance with the support plan.
In collaboration with participants, creates WRAP Plans and encourages the development of mental health advanced directives; facilitates peer support groups on participants' interests as needed.
Obtains necessary information during the intake process (if required), and collaboratively documents their progress in MHP's electronic health records system at the time of service; ensures that all data is correct by verifying the information with the participant.
Creates a weekly schedule, two weeks in advance that meets existing program productivity standards and supervision.
Supports participants in the community when participating in events, visits, and other interactions.
Complies with all other established quality and regulatory standards, including supervision and training requirements.
Maintains compliance with all MHP policies and procedures. Reports any activities that may violate established laws, regulations, policies or procedures. Raises questions about any actions contrary to law or policy taken by another staff member or employee or yourself, and reports the matter to management or to the Director - Quality Improvement & Compliance and/or Human Resources.

Qualifications

Minimum of a High School or GED Diploma; Associates Degree in a Human Services related field is preferred.
Credentialed as a Certified Peer Specialist through the PA Certification Board.
Within the last three (3) years, maintained at least 12 months of successful full or part-time paid or voluntary work experience.
Demonstrated knowledge of the local Mental Health System.
Must possess basic computer skills to perform job duties including desktop computing, e-mail, time sheet management, electronic health records, internet research, etc. and the ability to learn software applications relevant to your position.
Ability to work independently and to carry out assignments to completion within parameters of instructions given, prescribed routines, and standard accepted practices.
Strong skillsets in oral and written communication, cultural competence, interpersonal relationships and teamwork, attention to detail, customer service, time management, autonomy, problem solving and organization.
Must be able to work under pressure and meet deadlines and billable unit requirements, while maintaining a positive attitude and providing exemplary customer service.
Must possess a valid driver's license.
Must have access to a car to use for work on a daily basis with automobile insurance coverage at the state requirement levels and current state registration.
