We are currently searching for two Care Navigators. One for our 17th St. Outpatient Clinic in Harrisburg and one for our 128 N. George St. York clinic. Collaboration of systems and services is an important factor for increasing positive outcomes for individuals who are in treatment for opioid addiction. Care Navigators work directly with individuals with substance use disorder and provide support to bridge connections between treatment providers, health care professionals, insurance agencies and other services that may be beneficial to recovery. Overcoming addition is a multifaceted and individual process. Care Navigators work with individuals and service providers to connect clients to programs and timely assistance that will promote a recovery environment based on recommendations from treatment professionals. From a person’s entry into the treatment process through various possible programs and supports a Care Navigator is available to encourage, connect and support treatment plans.
Pay rate is $15/hour. Full time position with paid health insurance, paid time off and paid holidays.
